Please read the following carefully. Print out and sign your name at the bottom. If you are under 18, you must have your parent or guardian sign as well. Thank you.

Creative Edge Incorporated/Dance Collective; at the request of the subscriber, will provide a service of posting over the Internet a headshot (photograph), resume, and other personal information submitted by the subscriber. This information will be available to anyone viewing Dance Collective over the Internet. It is the subscriber's responsibility to judge whether the person, persons or business contacting you are legitimate and/or safe. Creative Edge Incorporated/Dance Collective will not be held responsible or liable for any damages or losses incurred (actual or perceived) by the use of Dance Collective.

As a subscriber, you will be permitted to submit one resume and one headshot. Each additional photograph will require an additional charge of $5.00. Any updates to your resume will be posted free of charge.

A headshot is defined as a 8"x10", black and white professional photo. Color photos will not be accepted.

All resumes submitted to Dance Collective may be edited in regards to format. The content of any resume submitted will not be changed. However, due to space limitations some information may be excluded.

Any photographs and resumes submitted to Dance Collective will not be returned.

Dance Collective does not guarantee auditions or employment.

There will be one dancer chosen each month to become our "Dancer of the Month". This feature entitles the "Dancer of the Month" additional exposure by displaying their headshot and biographical information on Dance Collective's main page, with a link to their resume. If you wish to be considered for this feature, you must complete the biographical information section at the bottom of this page. Completing this section in no way guarantees that you will be chosen "Dancer of the Month" at any time. The "Dancer of the Month" will be chosen at the sole discretion of Dance Collective. You must be a current/paid subscriber to be eligible for "Dancer of the Month".

Creative Edge Incorporated/Dance Collective reserves the right to remove at any time, for any reason, any information and/or subscriber's headshot and resume from Dance Collective without prior notice.

If the headshot and resume are removed from the website, by the request of the subscriber or Creative Edge Incorporated/Dance Collective, any unused time will be refunded on a pro-rated basis.

Dance Collective resides on a computer server not controlled by Creative Edge Incorporated/Dance Collective. At unknown times, the computer and/or various parts of the Internet may be unavailable due to repair, heavy Internet traffic, and/or other problems making your headshot, resume and other subscriber services unavailable.

The headshot, resume and other information the subscriber provides to Creative Edge Incorporated/Dance Collective is your legal right to use and is free from copyright, trademark, and ownership infringements. All information provided by you to Creative Edge Incorporated/Dance Collective pertains to you, the subscriber signed below, and no one else.

By joining or subscribing to Dance Collective, you acknowledge that you have carefully read and agree to the terms listed.

"Dancer of the Month" - Biographical Information
